STATUS: Non-Exempt / Part-time hourly position. Workdays may include weekends.

REPORTS TO: Executive Director


Position Summary

Habitat for Humanity of Carroll County seeks an organized and community-oriented Program Coordinator / Case Manager to support Aging in Place, part of their Critical Home Repair Program.

The program seeks to collaborate with human services organizations to evaluate individual needs and provide critical home repairs, modifications and community services specific to each homeowner’s lifestyle to preserve their home and their independence.

Working closely with Habitat staff, volunteers, and community partners, the Program Coordinator builds trusted relationships with homeowners and applicants, conducts in-home assessments alongside construction staff, and coordinates referrals to and from partner organizations. This role is central to Habitat’s relationship-based model, ensuring housing services are coordinated with broader supports that promote long-term housing stability.

Compensation is $20+/hour commensurate with experience. This position will begin in June 2026 with an initial 1-year term and potential to grow into a full-time role with benefits.

Key Responsibilities

Program Coordination & Client Engagement

  • Coordinate outreach with community partners to increase awareness of Habitat’s Critical Home Repair and homeownership programs, prioritizing underserved populations.
  • Participate in in-home visits with the Construction Manager to assess needs, initiate client relationships, and understand household circumstances.
  • Coordinate application intake and review with the Family Selection Committee. Case Management & Service Navigation
  • Build trusting relationships with homeowners and applicants to identify needs beyond housing repairs and modifications.
  • Assess needs related to basic support such as transportation, food access, benefits, and health services.
  • Coordinate referrals to appropriate service providers and assist with applications when needed.
  • Conduct follow-up with participants to monitor well-being and support long-term housing stability.


Outreach, Partnerships & Referral Coordination

  • Develop and maintain relationships with Carroll County agencies and nonprofit partners to support coordinated service delivery.
  • Manage referral pipelines into Habitat programs (home repair, homeownership, volunteer opportunities) and to external supportive services.
  • Serve as a liaison to ensure seamless, client-centered coordination.
  • Support outreach to low-income and underserved populations through community presentations and events.
  • Participate in coalitions and community initiatives to strengthen collaboration.


Documentation & Program Reporting

  • Maintain accurate, confidential client records in accordance with policies and grant requirements.
  • Track program outputs and outcomes, including client engagement and referrals.
  • Support data collection and reporting to demonstrate program impact
  • Contribute to program improvement based on data, participant feedback, and community needs.


Internal Collaboration

  • Work closely with Construction staff to coordinate repair timelines and communication with clients.
  • Collaborate with the Office Manager/Volunteer Coordinator to align volunteer engagement with program needs.
  • Participate in team meetings and continuous program improvement efforts.


Qualifications

Required

  • Experience in social services, case management, community outreach, or a related field
  • Strong interpersonal and relationship-building skills
  • Ability to work respectfully with older adults and individuals living with disabilities
  • Excellent organizational and communication skills
  • Ability to maintain confidentiality and handle sensitive information
  • Experience working with low-income or rural populations
  • Knowledge of Carroll County service networks
  • Valid driver’s license and ability to travel within Carroll County

Preferred

  • Familiarity with aging, disability, or community health service systems
  • Familiarity with financial literacy education and pre-purchase housing counseling methods.

Physical and Work Environment

This position requires regular travel throughout Carroll County and includes home visits and occasional visits to residential construction or repair sites.
